Abstract:
Objective: The roleofinflammatoryadipokineshasclearmechanisticeffectsinthepromotionofbothDM2
and cardiovasculardiseases(CVDs),butitisunknowntowhatextentatherosclerosis-relatedinflamma-
tion mightpromotedefectsofglucosemetabolism.Thepurposeofthisstudywastotestthehypothesis
that insubjectswithatheroscleroticvasculardiseaseandnopreviousmedicalrecordoftype2diabetes
mellitus (DM2),thediagnosisofoccultimpairedglucoseregulation(IGR)isrelatedtotheseverityof
atherosclerosis, measuredasthesingleorcombinedpresenceofanhistoryofcoronaryarterydisease
(CAD), carotidatherosclerosis(Car-ATS)andperipheralarterydisease(PAD).
Methods: In apopulationof551subjects(440menand111women)withaprevioushistoryofatheroscle-
rosis, weinvestigatedthepresenceofIGR(includingbothimpairedglucosetoleranceandDM2).Totest
the correlationbetweenconventionalandnon-conventionalriskfactorsforcardiovasculardiseaseand
diabetes weusedlogisticandregressionanalysismodels.
Results: IGR wasmoreprevalentinpatientswithadocumentedvasculardiseaseintwoorthreevessel
districts comparedwithpatientswithonlyonesymptomaticdistrict(p = 0.016).Amongclassicriskfactors
we foundthatwaistcircumferencewascorrelatedneithertoIGRnortosymptomaticvasculardisease
extension. Bycontrast,adiponectinlevelwasindependentlyassociatedtovascularandglucoseregulation
status (p = 0.012and p < 0.001,respectively).
Conclusion: In subjectsaffectedbyatheroscleroticvasculardiseases,thepresenceofimpairedglucose
regulation isassociatedtothenumberofvasculardistrictsaffectedandtoareducedadiponectinlevel.
